What is TMG4ME?

TMG4ME offers patients personalized and secure on-line access to portions of their medical records. It allows you to securely use the Internet to help manage and receive information about your health. With TMG4ME, you can use the Internet to:

How do I sign up?

Patients who wish to participate will need to complete the consent form and either mail it to the address on the form or turn it in during their clinic visit. Once the consent form is processed, you will receive a letter containing an activation code and you will also receive an e-mail containing your medical record number (MRN). The information we will provide you will enable you to login and create your own user ID and password.

When can I see my test results in TMG4ME?

Your test results are released to your TMG4ME account usually within 1-10 days.

Some tests results are considered sensitive and are not released to TMG4ME.

If some of my health information on TMG4ME is not correct, what should I do?

Your TMG4ME information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 2 business days. Please note that TMG4ME should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

Can I view a family member's health record in TMG4ME?

Yes you can. This is called Proxy access and allows a parent (or guardian) to log into their personal TMG4ME account, and then connect to information regarding their family member. Complete a Proxy Consent Form and return it to one of our medical facilities to request access to this convenient service.

Can my spouse and I share one TMG4ME account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own TMG4ME account.

Can you send me a new access code as I have lost it, let it expire or did not receive it?

You may click on the Contact Us link and submit your request electronically or contact us by phone: 1-877-9TMG4ME (1-877-986-4463) after we verify your information, a new code will be sent via U.S. Postal Mail. Privacy issues prevent us from e-mailing a new access code to you.

How is TMG4ME secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Futher, TMG4ME uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with TMG4ME. Unlike conventional e-mail, all TMG4ME messaging is done while you are securely logged on to our website.

I was logged out of TMG4ME, what happened?

We aim to protect your privacy and security of your information. While logged into TMG4ME, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of TMG4ME. We recommend that you log out of your TMG4ME session if you need to leave your computer for even a short period of time.

My access code does not work, what should I do?

For your security, your access code expires after 30 days and is no longer valid after the first time you use it. If you still have problems click on the Contact Us link or contact us by phone: 1-877-9TMG4ME (1-877-986-4463)

Is my access code my user ID?

No, your access code is not your TMG4ME ID or password. You will use this code only once to log into TMG4ME for the first time. (The code will expire after you have used it or after 30 days). When you log into TMG4ME the first time, you will then be asked to create your own unique TMG4ME ID and password.
